items disappearing

When I ran for the middle I got a stone sword and when I clicked on it it disappeared.

It is a 1.3 glitch, you probably took it right after someone else took it!

Even with any chest it still does it.

no, not always the case. I was alone, and the chests had just refilled, and this happened to me.

Also a glitch. If you are alone and you have taken the chest but your items mysteriously disappear just go check the chest again. All of the items should be there. If they are not you might want to look closely at your surroundings. ¬.¬
