What's your worst fail moment?

Title says it all.
My most fail-ez moment is this :
I was hosting a server for friends using Hamachi. Then once they left and so, I decided to play MCSG on AU. When I connect it says Connection Reset. Then I remembered about the Hamachi issue with EU and such ages ago. So I tried in every way to uninstall Hamachi. When I was about to break down, I remembered that my client was actually 1.2.5 -_-

haha, my worst fail (that i can remeber) is: i once had full iron armor, diamond sword. i was telling my friend not to fall in lava, and i fell in lava

When I had a truce with Hystericallify and I should protect him, and I fell asleep for 2 minutes behind a tree... He died and I was like
